We collect information that you provide to us when you apply for membership in an individual or family dental discount plan. This information includes:

Identity Information: We collect information used to identify you and your family members, such as name, date of birth, and gender.

Contact Information: We collect information used to stay in contact with you, such as your mailing and billing address, email address, and telephone number.

Payment Information: We collect information used to pay your membership fees, such as your credit card number (we only store the last four numbers of your credit card) and bank account information.

How we use your information

Your Identity, Contact, and Payment Information are used to process your application. Once you become a member, your Identity, Contact, and Payment Information are used to provide you the services and support set forth in the Plan Description of Services and Membership Agreement. We do not sell your personal information.
